Incident report form instructions

Please provide your full name, email address, and contact information so that an Officer can contact you to verify certain information, ask you additional question and provide you with the incident report case number.

Anonymous reports are accepted and will be investigated to the best of our abilities. However if you choose to remain anonymous, Officers will not be able to follow up with you if additional information is needed. Furthermore, Campus Safety and Prevention Services may not be able to take action related to your report if pertinent information is missing. 

If you are unsure of any details requested on this form, leave those fields blank. Providing misleading or inaccurate information may slow the report filing process.
