Cancellations

To ensure fair access to appointments for everyone, Health Services requires sufficient notice when you cancel. A $25 fee applies for missed appointments and late cancellations.

How to cancel your appointment

If you won't be able to attend your appointment, let us know as soon as possible using one of the following options:

  • Use the cancellation feature in your appointment reminders.
  • Call the 24/7 cancellation line at 514-848-2424, ext. 3565 and leave a voicemail with your name, student or staff ID, and appointment date, time and campus (SGW or LOY).
  • Visit one of our clinics at GM-200 (SGW) or AD-131 (LOY).

Missed appointments and late cancellations

As of November 24, 2025, you will be charged a $25 fee if you:

  • miss an appointment, or 
  • cancel less than 24 hours before your appointment time.

If your appointment is on Monday, cancel by Friday at 12 p.m. to avoid fees.

Students: Fees will be charged to your student account.

Faculty and staff: Fees must be paid by debit or credit at the front desk, or by calling 514-848-2424 ext. 3565 (credit only) before booking any future appointments.

Medical records

You have the right to access your medical records, in part or in full. 

If you have a RAMQ card, you can also access all your test results through your Carnet santé.

To get a copy of your Health Services medical records:

  1. Complete the release of information (ROI) form
  2. Bring the form to the clinic, or email it to healthservices.info@concordia.ca.
    Let us know what part of the file you need (e.g., test results, specialist consults) and the timeframe for your request (e.g., last six months).

You'll need to present a valid, government-issued photo ID to access your records. 

Medical records often include a lot of information, including some you may not need (e.g., forms and questionnaires, immunization records, clinical notes from all clinic health professionals, all test results, specialist consultations, life habits, medication lists, etc.). We strongly recommend specifying what part of your file you need in your request. 

This process only covers Health Services records. To access records from other on-campus services (e.g., Mental Health Entry, Counselling and Psychological Services), you’ll need to submit requests directly to those services.

Health Services will not include documents that are from a third party (e.g., copies of files from other clinics, or documents that you shared with the clinic).

If you need to send your records to third parties (e.g. another clinic) Health Services will prepare up to two copies of your file. You'll need to complete an ROI for each recipient.

If you need more than two copies, you can list yourself as one of the recipients, and then send your records to others as needed.

When sending medical records to a third party, only share what’s necessary to protect your personal information.
