In the face of the COVID-19 pandemic, requests that were previously made in-person at the Birks Student Service Centre can now be made online. In very specific cases, an in-person appointment can be reserved. Below you will find key information and steps to follow to make these requests.

Though not a preferred option, cheque payments can be left in the Birks mail slot without an appointment. Entry is via 1453 Mackay street, and Security must be advised of the purpose of the visit. The cheque can be left in the mail slot right by LB 185 in the lobby of the atrium. There is a sign just beneath the mail slot, indicating its location.
Cheques must be addressed to “Concordia University” and the student ID number should appear on the “memo” line.
A receipt can be issued following the processing of your cheque payment, if desired.
The STM uses the Concordia ID card photo in order to produce the OPUS card. Please ensure that your ID card is created before submitting an OPUS request.
After 24 hours, the OPUS card can be ordered online and the STM will send it by mail.
To be eligible, a student must have a provincial (Quebec) home address saved in their MyConcordia Student Centre and be registered full-time in the current term (at least 12 credits). Note that wait-listed courses do not count towards enrolment.

Follow the instructions and make a payment of $15 to the STM via the OPUS platform
Official transcripts are available by mail and by courier (additional fees applicable if courier is chosen). In-person pick-up is not possible due to restricted access to campus.
Transcripts destined to other universities in Quebec will be sent electronically.
Official transcripts can be sent electronically to institutions outside of Quebec if the institution’s e-mail address is included in the request. Please verify with the institution that it will be accepted in this format. (Please note: One email address per request).
An appointment can be made in order to discuss your Quebec Residency situation or to present a sworn statement to a Commissioner of oaths.
Appointments can be booked via your MyConcordia portal under Student Services (Birks Appointment Booking)
If you would like a sworn statement to be witnessed for AFE purposes, please communicate with the Financial Aid and Awards Office to ensure you are preparing the proper documents before scheduling a Birks appointment.
Replacement diplomas: Please see our diplomas website for more information and for the necessary forms. You can submit your request and provide a sworn statement at Birks by appointment. There is a $100 fee for a replacement diploma.
Certified copy of diploma (degree conferred prior to May 2020): Your original diploma is required for this service. The Birks team will issue a copy onsite at a cost of $10 per copy.
