Claim forms for Extended Health, Dental & HCSA
- Use the Extended health care and HCSA claim form for all medical expenses and services.
- Use the Dental & HCSA claim form only for dental expenses.

Tax slips
- If you are a pensioner who worked during the 2025 tax year, your tax slips related to employment income are available in UNITY, and your tax slips related to your pension income will be sent to your registered home address by CIBC Mellon via regular mail.
- If you worked during the 2025 tax year but no longer have access to UNITY, your tax slips will be mailed to your registered home address.
- If you are a pensioner who did not work during the 2025 tax year, your tax slips related to taxable benefits are available in UNITY, and your tax slips related to your pension income will be sent to your registered home address by CIBC Mellon via regular mail.
