The Mathematics and Statistics Co-operative program is offered to undergraduate students who are enrolled in the BA or BSc Honours or Specialization programs.

Please complete this form and provide the requested additional information by uploading your documents before submitting by the deadline (March 1st). All information is required; your application will be considered incomplete if anything is missing and will not be processed.

Which specialization do you prefer?

A statement of purpose explaining why you are interested in co-operative education, what you know about cooperative education, and what contribution you could make to our program.

Additional Documentation required:

1. A curriculum vitae (CV) describing your academic, personal and professional achievements, including previous employment, and an employability skills profile illustrating any specific skills you may have. Of particular relevance to co-operative education are computer skills, linguistic and writing skills, interpersonal skills, and organizational skills.

2. A copy of your most recent CEGEP transcript or equivalent and, where applicable, an up-to-date university transcript from the university which you have last attended or are currently attending. (Student copies of transcripts are acceptable; at the end of the selection process we will obtain copies of official transcripts from Concordia University’s Admissions Office.)
