Survey on health and wellness: We want to hear from you

The National College Health Assessment (NCHA) survey will be sent to a random sample of undergraduate and graduate students. If you’ve been selected, you will receive an email inviting you to complete this survey.

Your feedback and opinions are very important to us. The results of the survey can help us improve campus health promotion and prevention services.

Students who complete the survey will automatically be entered into a random prize draw to win one $400 prepaid Mastercard or Visa gift card, or one of eight $50 Amazon gift cards.

The survey closes April 12. You’ll receive a friendly reminder about it before the survey closes.
