  • Students who received any letters pertaining to conditional standing from Student Academic Services, and/ or any other documents that might be needed during the advising session. Remember to bring a recent copy of your transcripts (records). This can be printed from your portal.

Student request form

The Student Request Form is used for such things as:

Late DISC, Late DNE, Late Course Change, Credit Overload, Extension Late Completion Deadline, Late Registration, Transfer of External Credit(s), Course Substitution, Remove Exemption, Retain Credit, Waive 24 Credit Rule, Receive Credit for INTE 290, Waive Residency Requirement, General Education Requirement, and other.

You can access the forms via your MyConcordia Portal. Select the "Student Administrative Affairs" option. You will find the forms in the UG Student Requests link. Information about how to apply is provided on that website. You can complete and submit several of the forms on line.

Concentration change

If you are already a student in the Faculty of Arts and Science and would like to change your program or add a second program to one in the Sociology and Anthropology Department (except for Honours), you will need to have completed at least 30 credits, a GPA of 2.0 or higher and at least 6 in the program you are requesting entry into, with an average of B- in those courses.

If you meet these requirements, please print out the Change of Concentration Form and fill it out. Once it is completed, you will need to bring it and a copy of your transcript to one of the Department's faculty advisors for final approval.

If you are currently in a faculty other than the Faculty of Arts and Science, you will need to go to the Birks Student Service Centre and fill out an application form. If you are from outside the faculty, but would like to add a minor in Sociology and Anthropology, you can still do so through our Department's Change of Concentration Form. Please print out the Change of Concentration Form and fill it out. Once it is completed, you will need to bring it and a copy of your transcript to one of the Department's faculty advisors for final approval. Please consult the advising schedule.

Students interested in adding or changing programs to another department will need to consult that department. Our department only can add or change programs within our department.
