Enter your informed consent information into a multiple choice question with "I agree" as an answer option. Place this question into its own question group at the beginning of the survey.
Text from Microsoft Word cannot safely be directly imported into LimeSurvey via the standard copy and paste method and it is therefore not recommended. Code may be added that can affect how your survey functions and produce unpredictable results. Type your questions directly or save the Word document on your computer as a "Single File Web Page" (e.g. File... Save as... Single File Web Page), then open the saved web page with a web browser (e.g. File... Open file...) and then cut the text from this "web page" and paste it into LimeSurvey.
Yes - you can insert images into your questions. It is recommended that you reduce an image’s size before adding it.
Survey results can be fully anonymous (default mode) or linked to the survey respondent’s details.
Although your survey will remain on the server for a year after it has ended, you should export the responses in .xls, .csv or .spss format and store them in a secure location.
