Only Visiting Researchers or Non-student Research Employee's are required to complete this form. Access management has changed and is now linked to Concordia's student registration system and Human Resources database so access will end when a student graduates, takes a leave of absence or an employee's contract ends.
Support documents should be uploaded here. Documents may be submitted to EV2.705 in person but the evaluation of your request will not proceed until we have all relevant documents. This process could take a week once all the documents have been received.
