Reporting an IT incident
Use this form to report a lost or stolen university-owned computing device (e.g., departmental laptop, tablet, smartphone, USB drive, portable disk) or personal computing device that may contain sensitive university data.
Provide as much information as possible. In addition to filling out the form, a police report must be made by the Concordia Security Department for all stolen/lost items.
Submit a separate form for each computing device that has been lost or stolen.
