Service Dog Registration

As per the provisions of the Animals on University Premises Policy (VPS-22), Service Dogs must be registered with Campus Safety and Prevention Services.

Procedure for Registering a Service Dog:

1. Submit your online form; 

2. Only if requested, provide medical certificate to Campus Safety and Prevention Services;

3. Once approved, you will be provided with a registration number.
