Insurance claims deadline is April 30

Reminder to faculty and staff with health, dental insurance provided by Desjardins Financial Security

 For reimbursement of health and dental expenses for the year 2012, faculty and staff must submit their health and dental claims for expenses (incurred from January 1 to December 31, 2012, inclusive) before April 30, 2013.

It is important to allow for mail delivery time when processing any claims, as claims must be received by Desjardins by the deadline. Claims postmarked April 30 that are received afterwards will not be processed.

Claim forms are available on the Human Resources website under Benefit Forms. Completed forms should be mailed to:

The certificate number is the employee's identification number and the health and dental contract numbers are Q055 and Q056 respectively.
