All faculty and staff with health and dental insurance provided by Desjardins Financial Security should be sure to claim all expenses incurred in 2010 (January to December inclusive) before April 30, 2011. Desjardins must receive the claims by April 30: those postmarked April 30 will not be accepted.
Claim forms are available on the Human Resources website under Benefit Forms. Completed forms should be mailed to:

The certificate number is the claimant’s employee identification number and the health and dental contract numbers are Q055 and Q056 respectively. All insurance related queries may be directed to the Desjardins hotline: 1-866-526-7566.
