Proof of good health

Proof of good health initially means the completion of a medical questionnaire attesting to your good health. Depending on your responses to the questionnaire, Sun Life may require additional information which could include;

  • the completion of a more comprehensive questionnaire;
  • information from your attending physician or the hospital; and
  • a medical examination

When it's not required

It is not required for enrolment in the Basic Life Insurance Plan.

Under the Optional Life and Dependent Life Insurance Plans, proof of good health may not be required if you apply for coverage within 31 days of the date:

  • you are first eligible to enrol;
  • you are legally married;
  • you obtain your first dependent child;
  • you get divorced or your spouse dies (subject to your having eligible dependent children at that time); or
  • your eligible dependents cease to be covered under the group insurance provided through your spouse's employment. Proof of loss of coverage is required.

When it's required

Proof of good health must be submitted if:

  • you applied for optional life and/or dependent life insurance coverage more than 31 days after the events listed above;
  • you apply for an amount of optional life insurance coverage for yourself that exceeds:
    • 2 x your annual base salary; or
    • $300,000

When proof of good health is submitted, coverage takes effect from the date your application is approved, provided you are actively at work on that day.
