Seat-belt
At a glance
An additional 10% of the amount payable, up to $50,000, in case of death.
Should you die after sustaining an injury that is covered under the plan and you were wearing a properly fastened seat belt at the time of the accident, the plan will pay an additional 10% of the amount that would otherwise be payable, up to $50,000.
This benefit also applies for any eligible dependents covered under the plan.
Note: Verification that a seat belt was actually used must be part of the official accident report or certified by the investigating officer.
Felonious assault
At a glance
An additional 10% of the amount that would otherwise be payable for a covered loss.
Should you sustain covered injury the plan will pay an additional 10% of the amount that would otherwise be payable for a covered loss if such loss results from another person's criminal act or riot - attempted or actually committed - that is:
- directed at you as a representative of Concordia University, or at Concordia University in its usual course of business, its property or assets;
- not a moving violation as defined under the applicable province or state motor vehicle laws; and
- not the result of an act by a fellow employee or a member of your family or household.
A criminal act or riot includes robberies, thefts, bombings, kidnappings, hijackings, larceny, snipings, murders, riots or inciting to riot. In any case, the legal codes of the jurisdiction where the loss occurs will govern.
Common disaster
At a glance
Benefits increased for your spouse's death to the same amount as the benefit payable for your death.
If you cover your family under the plan and both you and your spouse die from the same accident and within 90 days of the accident, the plan will increase the benefit payable for your spouse's death to the same amount as the benefit payable for your death.
Child enhancement
With the exception of death, all benefits payable from the plan are doubled with respect to insured dependent children.
Rehabilitation
At a glance
Benefits payable for up to two years, to a maximum of $15,000 per covered individual.
- If you sustain a covered injury, the plan will reimburse all reasonable and necessary expenses you incur for special training to become specifically qualified to perform an occupation for which you previously lacked the qualifications.
- Benefits are payable for up to two years following the accident, to a maximum of $15,000 per covered individual for any one accident. The plan will not pay for any room, board or other ordinary living, travelling or clothing expenses.
- This rehabilitation benefit is also provided to any eligible dependents who sustain a covered injury.
Repatriation
At a glance
Maximum amount payable: $15,000 per covered individual.
- Should you or an eligible dependent die as a result of an accident at least 50 kilometres away from home (normal place of residence) and within 365 days after the accident, the plan will reimburse the expenses incurred for preparing (for burial or cremation) and shipping the body home.
- The maximum amount payable under this provision is $15,000 per covered individual.
Family transportation
At a glance
Up to $15,000, subject to certain criteria.
Should you or an eligible dependent be hospitalized for a covered loss, the plan will pay an immediate family member’s visiting expenses to a maximum of $15,000, provided the following criteria are met:
- the hospital confinement occurs within 365 days of the accident;
- the hospital is at least 100 kilometres away from home (normal place of residence);
- the attending physician recommends the personal attendance of a member of the immediate family;
- the visiting family member takes the most direct route, by a licensed common carrier, to the hospital; and
- the visiting family member is the spouse, parent, grandparent, child (aged 18 or over) or sibling of the covered person confined to the hospital.
Education
At a glance
- For your dependent children:
Reimbursement of post-secondary tuition fees, up to 10% of your coverage or $10,000 per school year, whichever is lower - For your spouse:
$15,000 for expenses incurred for any professional or trade training.
The plan also provides education benefits to your dependent children and/or spouse should you accidentally die. This benefit is also available should this happen to your spouse, if you have family coverage under the plan.
For your dependent children
In the event of your or your spouse's accidental death due to a covered accident, each eligible dependent child who was enrolled in an institution of higher learning on the date of the accident will be eligible for the lowest of the following benefits:
- the tuition, excluding room and board, charged by the institution for each school year;
- 10% of your coverage for accidental death; and
- $10,000 per school year, for up to four consecutive school years, as long as the eligible dependent child continues his or her education.
An institution of higher learning includes, but is not limited to, a university, private college and a trade school.
For your surviving spouse
If you should die accidentally, the plan will reimburse up to $15,000 for expenses incurred to provide your surviving spouse with any professional or trade training your spouse has signed up for to independently support herself or himself. The actual cost of the training must be incurred within 30 months from the date of your death.
Home and vehicle alterations
At a glance
One-time cost alterations refund if you (or your eligible dependent) need a wheelchair.
Maximum of $15,000.
Should you or an eligible dependent lose both feet or become quadriplegic, paraplegic or hemiplegic as the direct result of an accident, and subsequently need a wheelchair to get around, the plan will refund the one-time cost of alterations to:
- your principal residence; and
- one vehicle.
These two benefits combined are payable up to $15,000, provided that:
- proof of payment for these alterations is given;
- the home alterations are made on the covered person's behalf, done by someone with experience in this area and recommended by an organization recognized for its support and assistance to wheelchair users; and
- the vehicle alterations are made on the covered person's behalf, done by someone with experience in this area and approved by the provincial vehicle licensing authorities.
Identification
At a glance
Transportation of an immediate family member to identify remains following an accidental death.
Maximum of $5,000.
The plan pays a benefit of up to $5,000 for the transportation of an immediate family member to identify remains if you (or your eligible dependent) have an accidental death at least 150 kilometers from home and a law enforcement agency requests such identification.
Funeral expenses
At a glance
Covers funeral expenses in the event of an accidental death.
Maximum of $5,000.
The plan pays a benefit of up to $5,000 to reimburse funeral expenses if you (or your eligible dependent) have an accidental death.
