Eligibility & enrolment
Who is eligible
You are eligible to participate in the Concordia Optional Accident Insurance Plan if you are:
- a full-time, permanent employee; and
- a member of an eligible employee group.
Your spouse and dependent children are also eligible.
You cannot be covered under this plan both as an employee and as a dependent of another employee of Concordia University. Likewise, where both parents are employees of the University, children can only be covered as the dependents of one employee, not both.
When to enrol
You may enrol anytime you want once you are eligible - typically, this is when you are hired. If you have any eligible dependents, you may enroll them at the same time.
Once you have covered a dependent child, all subsequent eligible dependent children will automatically be covered.
When coverage begins
Your coverage, and that of any eligible dependents, will begin on the day Benefits Services receives your application.
Changing coverage
If you subsequently wish to change your coverage or your beneficiary, you can do so (subject to the plan provisions and provincial legislation).
- Contact Benefits Services; and
- Complete a new enrolment form.
